BELFORM TOV ("Belform", "we") provides general and specialised medical practice and other health services, rents medical and other equipment, and provides business consulting. This policy explains how we handle personal data under the Law of Ukraine "On Personal Data Protection", the Fundamentals of Ukrainian Legislation on Healthcare and the GDPR.

2. Patients
We process identity data, contacts and health data (medical records, test results, diagnoses) to provide medical care. Health data is special-category data processed by medical staff bound by medical confidentiality.
3. National e-health system
Where required, medical records, referrals and prescriptions are entered into the national e-health system in accordance with the law.
4. Children
Data of children is processed with the consent and in the presence of a parent or legal representative, except where the law provides otherwise.
5. Appointment tool
The time picker on this website runs in your browser; we receive your choice only when you send the form.
6. Equipment rental
Renter identity and contact details, delivery address, contract and deposit. We do not ask why the equipment is needed.
7. Consulting clients
Business contacts and contracts; we do not access patient data of client clinics without a lawful basis and agreement.
8. Purposes and grounds
Provision of medical care (special-category processing by health professionals), legal obligations (medical records, tax), performance of contracts, consent.
9. Recipients
Laboratories, the national e-health system, insurers with your consent, authorities where required by law. We do not sell personal data.
10. Retention
- Medical records: for the periods set by healthcare legislation.
- Rental contracts: 3 years.
11. Your rights
You may access your medical records, correct data, restrict or object to processing, obtain a copy and withdraw consent where processing is based on it. We reply within 30 days. Complaints may be addressed to the Ukrainian Parliament Commissioner for Human Rights or an EU supervisory authority.
